In a continuing effort to serve our customers we are now providing two convenient ways of getting your prescriptions refilled.

• Option 1: Download, fill out, and fax the refill form back to the appropriate store location where you plan to pick up your prescription. Fax numbers for each store are listed on the form. If you feel there may be a question as to the number of refills you have it is always best to call and confirm that there isn't a problem with your order prior to picking up your refill.
